Health fairs offer a great opportunity for your company, organization, house of worship, and/or community event to provide information about Alzheimer's disease and the programs and services available through the Alzheimer's Association!
To request the Alzheimer's Association's participation at your next health fair, please complete the Health Fair Request Form and return it to:
